Welcome to our Patient Portal!
All new and existing patients can create patient accounts with us through the portal.
In the portal, you can:
- Update your contact information
- Sign consent forms prior to arrival to maximize treatment time
- View scheduled appointments
- Request appointments (Coming soon...)
- View your previous invoices
To access your account:
- If you are an existing patient and we have your email address on file, your account has already been created
- Click the Forgot Password link and use the password that is emailed to you from the portal to access your account
- If you are a new patient or we do not have your email address on file, click "Create an Account"
- Enter your contact information
- You will be sent an activation code via email
- Check your junk mail if you don't see your code.
